From to it was part of the third tier, until the first of many re-structurings of the league system. The last of these occurred in , which saw the Regionalliga Nordost reinstated.
The league was made up of 18 clubs, with two coming from the 2nd Bundesliga and six each from Mitte and Nord while the south only sent four.

In October , another reform of the Regionalligas was agreed. The number of leagues was again expanded to five. The defunct Regionalliga Nordost was reestablished and a Regionalliga Bayern was established.
The new system came into operation at the beginning of the —13 season. The number of reserve teams per Regionalliga was limited to seven.
The new leagues will consist of up to 22 clubs in their inaugural season, but will then have to be reduced to between 16 and 18 clubs.
The Regionalligas will not be administrated by the DFB, but rather by the regional football associations. With regard to reserve teams, initially only seven are permitted per league, however, this rule may be subject to change under certain circumstances.
Reserve sides of 3rd Liga teams are not permitted in the Regionalliga. The reorganisation of the Regionalligas, so soon after the last changes in , became necessary because of a large number of insolvencies.

Fourteen out of sixteen Oberliga Nord clubs qualified for the new league, only the bottom two teams were relegated to the two new Oberligas.
These two leagues were in turn disbanded in when the Oberliga Nord was reformed. With the league changes in Germany in , the Oberliga Nord was again disbanded and the level below the Regionalliga Nord in this region were the five Verbandligas.
This required a promotion play-off for this league winners as there were not five promotion spots available for their region. The "new" Regionalliga Nord was actually a reformation of the "old" Regionalliga Nord which operated from to in the same region but then as the second tier of German football.
Unlike the "old" Regionalliga, the new one allowed reserve teams to compete in it. After six seasons, in , the number of Regionalligas was reduced from four to two.
The clubs of the other two were spread according to their geographical location. Only the teams placed two to six were permitted to remain in the league.
With the introduction of the 3rd Liga in and of a third Regionalliga, the Regionalliga West , the league became the fourth tier of German football.
The following 18 teams fulfilled the various qualification criteria and were granted a license for play in the new Regionalliga Nord for the season.
